The 5004P-021 Perpetual split chronograph manual-wind watch, features a 37 mm case. This is a platinum example with silver dial, arabic numerals and platinum deployant buckle. Functions include moon phase, day, date, hours, minutes, second hand, perpetual calendar, and split chronograph.

Patek Philippe Ref. 5004 was first produced in 1995. This highly complicated wristwatch combines a split-seconds chronograph with the functions of the perpetual calendar and moon phase. The movement consists of 407 hand finished parts. Only 12 of these watches were produced per year. Production of this reference ended in 2009.
